Participants Needed!

Patients are needed to participate in a focus group and provide feedback about their experiences at our primary care offices. Participants will receive dinner during the group discussion, plus a $50 gift card. See below for more details.

The Rhode Island Chronic Care Sustainability Initiative (CSI-RI), RI’s patient-centered medical home initiative, is hosting a patient focus group to discuss patient primary care experiences. BVCHC is a participating member of the CSI-RI and we’d like to hear from you! The focus group will be held on:

Tuesday, September 30, 2014
5:00 – 7:30pm
Chelo’s (Cranston location)
1275 Reservoir Avenue, Cranston RI 02920
